Working in partnership to develop local Population Health Management (PHM) capabilities
Working with NHS England, Optum supported 36 ICSs to deliver the PHM Programme. We supported NHS leadership teams to create change management strategies around the following:
- Workforce
- Finance
- Digital transformation
- Health inequalities
- Evaluation
Since completing the PHM Development Programme, our expert team of analysts, clinicians and programme directors has worked with ICS system, place and neighbourhood teams to link datasets and surface insight. Our NHS customers have been able to bring together groups which previously worked independently, such as analysts and clinicians, and change the way they view patients and pathways. Local teams are building care around a person rather than managing diseases.
Linked datasets can be visualised on our PHM analytics platform. The platform enables a local shared understanding of the impact of the wider determinants of health across an ICS. Local care teams can adapt services to meet the needs of communities and the people they serve.
Using our range of PHM products and services, Optum supports customers to use integrated data to effect change. Our teams enable ICSs to create more effective interventions and assess the impact of changes made. Using our PHM analytics solutions customers develop PHM segmentation models to identify and support at-risk populations to achieve better health outcomes, as well as tracking cohorts to project future outcomes.
